Background Checks and Right to Review

• • LAPS Authorization Form  – to be used for agency records when using the LAPS program
  •  Authorization Form – to be used in conjunction with the appropriate Disclosure Form when requesting background checks with fingerprint cards.
  • General Disclosure Form – to be submitted when an employer is requesting a background check on an employee.
